How do I add a rating scale question to my Microsoft Forms form?

1. Click on the + Add new dropdown button next to any existing questions.
2. Select Rating.
3. Enter your Question text in the box.
4. Select the number of Rating levels you would like to use.
5. Pick a Style for your rating scale.
