To remove the title and description from a Microsoft Forms form, you will need to do the following:

1. Navigate to the Microsoft Forms homepage and click the form you wish to edit
2. Click the Edit button in the top right corner
3. Click Design
4. Clear the Title and Description fields
5. Click save.
